POSITIVE LIVING BC wishes to appoint a Peer Navigator.

The Peer Navigator assesses member/participant needs related to health, wellness, social and emotional well-being, and, in conjunction with the member/participant, develops strategies to maintain or improve health and wellness needs and to implement self-management skill-sets.

Working collaboratively with other healthcare providers, the Peer Navigator:

• provides direct system navigation, advocacy, education and related linkage to relevant specialized health and wellness resources related to HIV
• networks with front line service providers and builds collaborative inter-agency partnerships in order to promote Peer Navigation Services
• addresses issues related to people newly diagnosed with HIV or seeking to develop HIV related self-management skills.
